Boeing CEO Dennis Muilenburg says the company made mistakes with the 737 Max.

During testimony before Congress, Washington representative Rick Larsen had this question: “Can you name three specific mistakes?"

Muilenburg started out with the optional indicator, which could have helped pilots identify a misfiring flight control system.

"Implementation of the angle of attack disagree alert - we got that wrong," he said.

Many airlines didn't have that indicator installed.

"Secondly we've learned about the MCAS architecture," he said.

MCAS is the flight control system that pushed the plane's nose down in two crashes, overpowering the pilots.

"And thirdly, communication, documentation ," he said, in regards to the failure to tell pilots about this system until after the first plane crashed.

Muilenburg said multiple organizations, including outside Boeing, are responsible.

He says Boeing may not be done reforming its internal culture.
